Foster Agreement

I, the undersigned, hereby confirm that I am providing volunteer services to The Lizard Lighthouse Inc., having been made fully aware of the risks involved. For purposes of this agreement, “volunteer services” include, but are not limited to, fostering animals, relocating animals, transporting animals, assisting with adoption events, caring for The Lizard Lighthouse Inc. animals, and any other activity relating thereto and associated with The Lizard Lighthouse Inc. I fully understand and agree to assume all risks involved in any duties that I perform for The Lizard Lighthouse Inc. in my volunteer capacity, and I agree to hold The Lizard Lighthouse Inc. harmless for any injury(s) that I might sustain during the course of my volunteer duties.

I understand and agree that neither The Lizard Lighthouse Inc. nor third-party locations are responsible for any illness or injury caused by any animals that I come into contact with during my foster/volunteer work. I hereby release, waive, discharge, and covenant not to hold responsible The Lizard Lighthouse Inc. and its officers, agents, other volunteers, servants, or employees from any and all liability, claims, demands, actions, including court costs and attorney fees, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of the property belonging to me.

I agree that I am able and willing to follow any and all care instructions that have been provided by The Lizard Lighthouse Inc. I have access to and the ability to obtain services from an exotic veterinarian. I will not receive any animals or supplies on behalf of The Lizard Lighthouse Inc. without The Lizard Lighthouse Inc.’s knowledge and consent. I understand that this is not a paid position and that I am limited to fostering up to two (2) animals at any time. In the event that I am unable to continue caring for an animal, I will contact an authorized representative of The Lizard Lighthouse Inc.

I understand that The Lizard Lighthouse Inc. is the owner of the animals, supplies, and equipment provided. I will return any animal, supplies, or equipment provided upon request of an authorized representative of The Lizard Lighthouse Inc. I will return any animals, supplies, or equipment to The Lizard Lighthouse Inc. in substantially the same condition in which they were provided. If any supplies or equipment are not returned to The Lizard Lighthouse Inc. in substantially the same condition in which they were provided, I agree to replace said supplies or equipment or provide funds to The Lizard Lighthouse Inc. to replace the supplies and/or equipment. The following equipment is being provided to me to care for the foster animal.

In the event I agree to foster or transport animals for The Lizard Lighthouse Inc., I agree to voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or any loss or damage of property owned by me, as a result of fostering or transporting said animal, whether caused by the negligence of The Lizard Lighthouse Inc, its officers, agents, other volunteers, servants, employees or otherwise.

It is my express intent that this Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns, and personal representative, if I am deceased, and shall be deemed as a release, waiver, discharge, and covenant not to hold responsible The Lizard Lighthouse Inc, its officers, agents, servants, or employees. I hereby further agree that this Hold Harmless Agreement shall be construed in accordance with the laws of the State of Nebraska.

In signing this agreement, I acknowledge and represent that I have read the foregoing Hold Harmless Agreement, understand it, and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least nineteen (19) years of age and fully competent; and I execute this Agreement
